Physical Examination

Must be completed for Kindergarten, 6th Grade, and any student entering CPS for the first time.
Vision Examination Report
by October 15th
Must be completed for Kindergarten or if you are entering the state of IL for the first time for ANY grade.
Dental Examination
by October 15th
Must be completed for Kindergarten, 2nd, and 6th
Student Fees
These are paid by all students, and are used to supplement the budget supplied by CPS and allow us to continue the opportunities we have provided in the past included below and more. Just as they were last year, student fees are $100. Families will be able to pay via e-pay with the link below, or by cash or check in our main office.
Student Fees Are Used For
- Updated technology (Chromebook carts for classrooms)
- Classroom resources (leveled student texts, class novel sets, science materials)
- Classroom subscriptions (Time for Kids, Social Studies Weekly, Science World)
- Online subscriptions (IXL Math, Freckle, Reading A-Z)
- Interactive whiteboards
- Student planners
- Fine arts materials including musical instruments
- Academic workbooks and other consumables
- Copier use for student assignments
