2-Step Registration Process

Edison Park Elementary School is a neighborhood school and we are not opening seats to students who live outside our attendance area at this time.

If you are interested in registering your child at EPE, please verify you live within our boundaries and follow through to step 2. Please verify if you live in EPE’s attendance area.

Step 2

Complete Google Form With Docs

Please know that you must have all documents ready to upload as you fill out the Google Form. You will not be able to submit unless you have all the proper documents uploaded.

Note: If you’re new to CPS, please disregard any sections that ask for an ID#.

As you fill out this form, you will be required to upload files. We recommend two apps to help scan and create pdfs:

Section 2A – Required Documents

Provide the appropriate documents and download, fill out, and scan all health forms below.

Driver’s License

Please provide a copy of your license that shows your current address within our neighborhood borders.

Second Proof of Address

Please provide documentation that MATCHES YOUR DRIVER’S LICENSE, such as…

  • Utility Bill
  • Recent Mortgage Statement within the last two months
  • Current notarized lease/rental agreement
Child’s Birth Certificate

Please provide a copy of your child’s birth certificate.

Section 2B – Required Medical Documents

Download, fill out, and scan all health forms below. Medical Records (dental, vision, and physical) are due the day a newly enrolled student begins school. We will accept medical records sooner. Please click here ​​​​​​​for more details on which medical records are required.

Physical Examination
by October 15th

Must be completed for Kindergarten, 6th Grade, and any student entering CPS for the first time. Physical Examination

by October 15th

Must be completed for Kindergarten or if you are entering the state of IL for the first time for ANY grade. English Spanish
